Referrals

 
Appropriate referrals include students ages 14-22 who:
  • Are currently on an Individualized Education Program (IEP)

  • Present with moderate to significant cognitive, communication, and/or social needs

  • Require moderate to significant modifications to the curriculum to make effective academic progress

  • Exhibit challenging behaviors that interfere with learning (e.g., aggression, self-injury)

 

Students attending SEEM Prep may have a range of developmental, neurological, or emotional/behavioral challenges. These may include:

  • Complex developmental disabilities

  • Autism and related social-communication needs

  • Neurological or genetic conditions

  • Emotional or behavioral regulation needs

  • Co-occurring mental health or medical needs

 
 

Referral packets should include:

  • Cover letter from the district

  • Current IEP

  • Most recent evaluations

  • Functional Behavioral Assessment (FBA) and Behavior Intervention Plan (BIP),  if applicable

  • Incident reports, if applicable

 
 
Referrals to SEEM Prep must come directly from the student’s sending school district.
